We help to make sure crucial information is conveyed to the rehab facility and that you fully understand your discharge instructions. When you or your loved one is released from the hospital additional care may be necessary - either at home, in a rehab facility, or in an assisted living community. All patients are vulnerable to the potential effects of medical errors. Risk factors such as age, complex care, urgent care, and a prolonged hospital stay can increase the chances for medical errors.
